IN NETWORK INSURANCES
If we do not accept your insurance, we will be able to provide you with appointment receipts/a superbill once a month, so that you may seek potential reimbursement
through your insurance on your own.
Please note: We can not and do not guarantee reimbursement.
Co-Pays and Co-Insurance are based upon copay or deductible.
This fee is due at the start of service.
FORMS OF PAYMENT
AG Counseling and Consulting, LLC accepts all major credit cards, cashapp/$AGCCLLC and health savings debit cards. Payments are due at the time of service and can be processed via the patient portal. A credit card form will be sent to you with your intake documents to fill out prior to your first session. You are responsible for payment of your session if you lose coverage and/or your insurance does not pay for the claim. We do not accept checks.
MISSED APPOINTMENT/CANCELLATION POLICY
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
Clients will not be able to rebook until this charge has been cleared.
